About Modularis
Modularis is a mobile app for collecting, analyzing, and providing patient-reported information (PROMs) and other health-related data. The app is being used as part of the Modularis study and is currently aimed specifically at liver transplant patients. It accompanies patients before and during the transplant and helps them actively shape their health.
Core functions:
Support during the transplant phase: Modularis provides targeted information to guide patients during this sensitive phase. This includes medical content, procedures, and advice on managing the disease and treatment.
Medication tracking: The app allows you to record personal medication information and receive reminders for medication times when needed. This way, you always have an overview of your medication.
Recording PROMs: Users answer validated, standardized questionnaires on quality of life, symptoms, or functional limitations. The app reminds users of upcoming entries and adapts content to the respective treatment time.
Progress & Analytics: A dashboard visualizes changes over time, e.g., in physical or mental well-being. These insights enable individualized care and promote collaborative decision-making.
