Medibank Heart Health at Home is a phone-based cardiac rehabilitation program that runs over 6 to 8 weeks and you can participate no matter where you live in Australia. Cardiac rehabilitation programs have been shown to reduce the risks of a heart attack and may prevent the need for more cardiac surgery.
You’ll receive:
• Personalised one-on-one weekly coaching sessions with an experienced health professional
• Support and counselling to make lifestyle changes and reduce stress
• A program tailored to increase your physical activity
The program is supported by this evidence based mobile digital application which provides advice and guidance to help you manage and keep track of your blood pressure, stress, medications, weight and other risk factors.
