About SRIA

The Anesthesia Incident Reporting System (SRIA) was implemented by the Anesthesiology Society of the State of São Paulo (SAESP) with the objective of reinforcing the initiatives to promote Patient Safety.

SRIA is the result of a partnership between Anesthesia Quality Institute (AQI) and SAESP. AQI, the creator of the incident reporting system (AIRS), ceded the entire system free of charge, in addition to sharing technological knowledge and experience gained from AIRS.

Information submitted in SRIA is transmitted to a server securely, encrypted and kept under external access protection, thereby ensuring the confidentiality of the information. In addition, SAESP is legally unable to disclose the institution, patient or physician involved in the reports received. In reports submitted in a confidential manner, a member of the SAESP Patient Safety Committee may contact the notifier to request additional information and follow up on the analysis, then provide guidance and clarification. A reference number provided may be used in subsequent revisions.
