The Empower Patient Support Program app enables Jardiance® patients to enroll into the Empower Patient Support Program and access program features including:


- Videos and articles about your health condition
- Videos and articles about Jardiance®
- Medication reminders
- Chat with program staff

Please have your Jardiance® DIN number handy when creating your account. You can locate this from your Jardiance® package (8-digit number) in the top right corner.
