A fast and convenient online form for citizens to file complaints about violations that have harmed their rights, legitimate interests, life or health, the environment or the security of the state, with the addition of documents or copies confirming such violations.
The online application form consists of 5 blocks:
"To", "From whom", "Complaint / Statement / Proposal", "Appendix", and "Your signature".
Fields of all blocks are required.
In the block "Complaint / Statement / Proposal" you need to describe the essence of the problem, indicating the legal name of the entrepreneur and his actual location.
The "Appendix" block is designed to provide evidence of the event, namely:
scanned copy of the goods or cash receipt,
photo of low-quality goods.
The block "Your signature" is required to sign the appeal, which is submitted to the Main Department of the State Food and Consumer Service in Kyiv, filled in with your finger on the screen (for a smartphone or tablet).
The date of application is set automatically by the system.
After clicking the "Send" button, the appeal will be sent to the State Service for Food and Consumer Protection in Kyiv.
