This charter has been signed by all regions of France, and brings together 12 action points to improve the care of people living with a disability in the healthcare system.
The questionnaire is made up of twenty simple questions written in Easy to Read and Understand (FALC), and is intended to be completed by the patient at the end of each appointment with a healthcare professional.
These data, completely anonymous, allow after analysis to identify the problems remaining within the care path and help the Handidactique association to adapt its actions to more specific problems.
The results are published quarterly on the app and on the Handifaction website.
