“Looped In” is a tool meant to support families of children and youth with special needs, to better organize their appointments/referrals and better understand who is on their team. It provides local agency information and links for general inquiries as well as staff names, roles and phone numbers so they can easily be found by families. Families enter the information about services they are waiting to receive, are receiving or have completed, as wellas their child’s team members/service providers. This app is part of our Coordinated Service Planning efforts for families in theKingston, Frontenac, Lennox & Addington and Lanark, Leeds & Grenville regions.
