MODEP SMART UP

As a MODEP member, the MODEP SmartUp app brings you closer to your mutual, giving you all information about:
   - Situation of medical records (outpatient and outpatient care)
   - Family situation and benefits
   - Assistancy

Also the mobile application provides you with an information center containing:
   - Downloadable documents
   - Partner service providers
   - The administrative documents

And finally, via MODEP SMART UP, you will be notified of any news, whether it is for the follow-up of your personal files or for general information and news concerning the activity of your Mutual.

To create your account, fill in the required fields:
   . The NAX: the last 6 digits of the registration number
   . The number of the CIN (the first 2 letters in capital letters)
   . The telephone number (the one communicated to the mutual society)
   . The identifier (to connect) is filled automatically by the Nax but you can modify it
   . Enter the password twice
