Your Care ID will allow you to access:
• NHS national systems
• clinical data
• patient information
This app will take a photograph of your documents and face.
If you chose to opt out of verifying your address with TransUnion, you may need 2 current photo IDs from the following:
• passport
• driving licence
• UK Biometric residence permit card
• HM armed forces identity card
And you will also need 1 proof of address from the following:
• Bank or credit card statement from the last 6 months
• Mortgage statement from the last 12 months
• Utility bill, including gas, electric, water or landline telephone from the last 6 months
• Council tax statement from the last 12 months
• UK driving licence (this cannot be used as both photo ID and proof of address)
