HOPE (Healthy living with Online Support & Education) is an e-counselling digital intervention programme that supports healthy lifestyle through behaviour changes. It incorporates psychological framework that guides users at different stages of behavioural change with a series of modules over 24 weeks. The components of the programme include videos, text content, interactive tools, and trackers.
The purpose of the program is to:
- To create an effective chronic disease prevention programme that is scalable to reduce the incidence of cardiovascular disease (CVD) in Singapore.
- To promote the adoption and maintenance of self-care behaviours, including maintaining a healthy diet, regular exercise, and medication adherence to reduce the modifiable risk factors for CVD.
