The Incident Reporting (FIR) And Investigation system(IRIS) is workflow based application developed mainly to record and to do root cause analysis of every Incidents Involving Damage to Plants and Equipment’s, Injury to Others as well as Incidents Not Causing any Damage or Injury but having potential to do so (Near Miss). Incident Reporting (FIR) and Investigation system is built-in for reducing Incident Registration and Investigation Time and to capture ATR and its timely closure. With the above objective the system is redesigned to bring both technical and functional enhancements. The Key features of new system are the System is integrated with other system like Employee Health Management System (HIMS), SAP, Contractor Pass Management, Work Permit System etc. The system has configurable workflow and can be easily configured for other location/Refinery rollout Incident Registration and Investigation is done by defined roles/assigned user in system and at each step time line is assigned in system. This will reduce incident registration and investigation time by auto reminders thru system.
