This application allows the nurses at UOHI to extend their care and help monitor patient after they have been discharged from the hospital. It will track the recovery process of each patient and help them with a speedy recovery. In order to be eligible for this type of monitoring the patient would need to have received a surgical procedure from the University of Ottawa Heart Institute.
