Continua Mobile is the IOS and Android application of the CONTINUA home monitoring service. A patient-friendly service that, thanks to human technologies and processes, will be able to bring the best socio-health services of the reference Healthcare System to your home. The CONTINUA service will accompany you in your daily activities by providing all health professionals (General Practitioner, ADI, Care Giver, Health District, Reference Specialists, Casa Della Salute) with the data necessary for a targeted and proactive management having as common objective a improving your health.

The application can be used in two different ways depending on your skills and needs, it will be she who will adapt to you and not the other way around. Do you consider the App too complex a system? The Continua service will contact you by phone according to the care plan set for you and will guide you in the detection of clinical parameters. For your greater protection, all activities registered through the Continua Mobile app will be viewed by our Clinical Center which, if it highlights a need, will activate the right processes for taking charge. The Continua Mobile App is not a Teleservice system and does not in any way replace the 112 Emergency Health service. All personal and health data will be treated in accordance with current Italian and European regulations.
