"My ToMMo" is an app provided by the Tohoku University Tohoku Medical Megabank Organization to those who are subject to the cohort survey.
■ Registration
The following items are required when registering to use My ToMMo.
・ My Number Card: Please apply by yourself
・ My Number Card PIN: A long PIN and a short PIN are required.
・ Additional cohort number: Issued by ToMMo
・ Smartphone that can read my number card

[Electronic circulation of health survey results]
You can view the results of ToMMo's health survey, which was previously delivered on paper, on your smartphone. You can also send an e-mail, so you can print it by sending it to an e-mail address that can be received on your computer. Please use it for your own health management and confirmation on the go.
[Application for viewing family health survey results]
With My ToMMo, you will be able to see the results of your family's health survey in the same app.
* To view the results of health surveys by anyone other than the person who is logged in, the approval of ToMMo, the administrator, is required.

You can check the notification from ToMMo.
[Record management of maternal and child health handbook]
Infant medical examination results and vaccination records recorded in the paper Maternal and Child Health Handbook can also be recorded on the app. You can rest assured that you will be able to manage the information that your child will need when they grow up in the future with data.
■ Notes
[About My Number Card]
・ Keep holding my number card until reading is completed.
・ If the response is poor, remove the cover or case and try again.
-If the holding position is not appropriate, it cannot be read.
[About PIN (password)]
・ The short security code (user authentication password) is 4 numbers.
･ The short security code is locked if you enter it incorrectly three times in a row.
Please contact your local municipal office for how to unlock.
[About digital certificates]
・ When applying for the Individual Number Card, check the "No digital certificate for signature" or "No digital certificate for user certification" column, and if you have not issued an electronic certificate, use it until the electronic certificate is issued. I can't.
・ Digital certificates that have expired and digital certificates that have expired due to moving, etc. cannot be used until they are renewed.
・ For information on how to renew and issue a digital certificate, please contact the municipal office where you live.
