Formas365 enables the physician to send important forms to their patients and ensures that they receive them, sign them and return them digitally.
The formats can be letters of communication about the clinic service, health questionnaires, surveys of the service provided in the doctor's clinic and any other document that is important for the patient to receive and sign accordingly.
The operation of Formas365 is very simple. Once the doctor downloads the application and registers with his data, he immediately captures the general data of his patients, then registers the appointments of his patients and selects the document format to send.
Sending to patients is done by emails or WhatsApp text message.
The patient receives the message with a link which opens for you to review or answer the form, at the end sign it and send it to him.
The form is received by the doctor in his APP FORMS 365. If no response is received from the patient, the physician can resend the format to the patient.
