PatientCoach

PatientCoach consists of an app for patients and a web-application for professionals. PatientCoach enables obtaining outcomes directly from patients and in addition supports telemonitoring and self-management for patients with one or more chronic disorder. PatientCoach is developed for several clinical studies in the Netherlands for patients with asthma, COPD, hypertension, diabetes and/or obesity.

PatientCoach use is not freely available, but can only be used in conjunction with an existing patient account created by a professional for a specific project, see for more information at www.patientcoach.lumc.nl.

So far, this app can only be used in a study context.
