NXT HealthCenter gives you control over your personal health information. This app will enable you to view, download, organize, and share health information for your own use or to share with your doctors and other trusted individuals.
You will have multiple options to either upload or populate your health information into your HealthCenter account. You may manually input information into the system from your personal health documents or records. Alternatively, you may choose the option to receive personal health records via a secure communication directly from your personal care provider. Provided those records are in an accepted format (e.g., .xml, .jpg), you may directly upload the information from those records into your HealthCenter account.
This application only works for users with an active HealthCenter account.
