Risk Assessment Tool lets you create and manage simple tests.

The tests you create can contain simple yes/no questions, numeric questions, or text input fields. For example, the FAA FRAT give a 10 point credit if the pilot is IFR rated (a yes/no question), or you could create a numeric question where if the pilot has less than 25 hours in type they score 5 points; less than 50 they score 3 points, etc.
You can email a PDF of test results to other people to view -- like your flight instructor, chief pilot, or operations department.
Tests you create can be published for others to download and use. You can share your tests publicly or privately.
Sharing and downloading other people's tests require an in app purchase available as either a monthly or yearly subscription. Purchase will be applied to your iTunes account on confirmation. Subscriptions will automatically renew unless canceled within 24-hours before the end of the current period. You can cancel anytime with your iTunes account settings.
