Care Team Connect is designed to help care organizations collaborate with families of people living with dementia to incorporate meaningful engagement activities.

Using Care Team Connect, participating care organization are able to create engagement profiles for clients that are receiving care services. Upon receiving an invitation from the organization, the client's family members are prompted to add meaningful photos, and recommendations for conversation topics and music preferences to the client's profile.
