EHI Health is a custom solution for Patients who participate with Remote Patient Monitoring (RPM) program. Our App allows patients the ability to keep record of their vitals, such as blood pressure, blood sugar, pulse oximeter, and weight by syncing with FDA approved iHealth products and securely transmitting that data directly to our providers. RPM is a CMS initiative that allows providers monitor your vitals with accuracy and ease, all from your home. Using this data, EHI providers have the ability to apply analytics to make better medical decisions and adjust care plans for improved quality of life for our patients. Combining RPM with our Chronic Care Management program also allows practices to bridge the gap between office visits with monthly review. This provides you a better relationship with your provider giving them more information and meaningful interpretations of your health.
