To start using the Track Rehab app, please ask your therapist to create an exercise plan on the Rehab My Patient app or website.

Track Rehab is part of the Rehab My Patient software. Rehab My Patient is exercise prescription software that's used and loved by many clinic's around the world. Main purpose of this website is to allow patients of clinic's that use Rehab My Paptient to access exercise plans that are prescribed for them. This website also have RMP Patient Diary that allows patients to track their pain rating's and how often they do exercises.
