The Burlington Health Navigator app helps patients, families and caregivers discover and connect to local health and social care resources to support their well-being. As a digital tool co-designed with patient partners, users are encouraged to engage with the Navigator content and provide feedback to further enhance future releases.

Burlington Health Navigator's latest release includes integration with the new ConnectMyHealth patient portal. We have also inlcuded direct links from the homepage to Health811, where you can get free health advice or find health services over the phone or online 24/7.
