PRISM is a virtual care program designed to support patients on their health recovery journey. Developed by experts at Mount Sinai Hospital in New York City, one of the nation’s leading medical centers, PRISM is now available to patients and their care partners across the country.
The PRISM App allows users to seamlessly connect and share health information with a dedicated PRISM Care Partner — a healthcare professional who provides guidance, encouragement, and personalized support throughout recovery.
Patients who have enrolled in the PRISM program through their hospital or the PRISM website (myprism.ai) will receive secure login credentials to access the app. Once downloaded, the PRISM app makes it easy to:
- Complete enrollment steps
- Create a personalized patient profile
- Capture and share blood pressure and heart rate data
- Stay on track with daily to-do lists
- Access helpful health education resources
- Monitor progress and key recovery milestones over time
- Schedule and join appointments with Care Partners
- Have video visits with Care Partners directly through the app
