MyCareHalo App allows users to record and share health information with healthcare providers. MycareHalo App is a mobile remote patient monitoring application that allows patients to measure and track her/his vitals using provided medical devices.

This application is for information only and does not serve as a medical diagnosis. Members should consult their referring healthcare providers with any questions or concerns.

Membership requires enrollment into the program through your health plan or healthcare provider. Please contact your healthcare provider to request an account. This application supports reading collected data from myCareHalo App provided connected Bluetooth devices. Member devices will be determined by your healthcare provider. Device options include blood pressure, blood glucose, thermometer, pulse oximeter, and body analyzer scale.
The vital information collected in the app will be made available to your Healthcare Provider. You can see your collected vital information in the History section of the application.

The myCareHalo App is not for emergency use. Always consult your healthcare providers before beginning any exercise program. Please consult your healthcare providers for medical interpretation of the data and medical advice.
