Welcome to the myLTFU portal app! You can use this app to securely complete your follow-up surveys in the LTFU Study and stay connected to the research team.

The Long-Term Follow-Up (LTFU) Study is a research program that aims to improve the long-term health and quality of life of childhood cancer survivors. By participating in the LTFU Study, you are helping researchers better understand how cancer and its treatment affect people years later—so that future survivors can live longer, healthier lives.

Participation is by invitation only.

With this app, you’ll be able to:
- Complete surveys about your health, treatment history, and life after cancer
- Share your electronic health records (EHR), including information about doctor visits, medications, and test results
- Receive study updates, helpful resources, and opportunities to participate in future research
- You will also have the opportunity to share your electronic health records (with optional selections for Apple's HealthKit and Clinical Health Records)

All data collected is stored securely to protect your privacy.
