To help manage long term conditions, Stoke-on-Trent Clinical Commissioning Group and the School of Pharmacy at Keele University have developed an application to support patients to manage their condition. Content packs available for:

- Asthma
- COPD
- Type 2 Diabetes
- Hypertension
- Stroke/TIA
- Atrial Fibrillation

With more being developed or in the process of being updated.
