Integrate

Integrate will support mobility for CareDirector users, both online and offline, to enhance the service provided to patients and people in need.

This App will be used by social workers and health workers to support care delivery in home and community based settings.

The long term vision is that the App could in the future be used by patients and people in need themselves to actively participate in the decisions around their own care.
