The CareSimple Patient App is an easy way for you to monitor and manage your health conditions and your communications with your care team from the comfort of your home.
The application should be used in conjunction with your healthcare professional who will check your data to make medical decisions.
The application must be used only in the USA and Canada because it is regulated by the FDA and Heath Canada, respectively, as a class 1 medical device.
HOW IT WORKS
Your doctor sends you an invitation to participate in a CareSimple Remote Patient Monitoring program, and you’ll receive a code to access the app as well as a device in the mail. This device takes your measurements and sends your readings directly to your health file in your physician’s office.
Your care team then monitors your health and can make informed decisions on how to provide the best care for you and your condition based on the regular readings you provide.
It’s that simple!
The CareSimple Patient App is your companion app throughout the program. Use the code your physician provides to you to create an account that will help you with the following:
CARE PLAN
A personalized care plan will be created by your health care provider. The plan will include measurements (ex: take blood pressure daily), questionnaires (ex: respond to wellness check monthly) or other instructions (ex: check your feet weekly). The app will remind you when it’s time to act with a push notification, and by showing active “Tasks” on the home screen. All you have to do is tap and follow the on-screen instructions. Tasks typically take a minute or less.
RESULTS
The info collected in the app will be available to your health provider. You can also see the information in the “Results” section, which uses graphs and color codes to add meaning to your data.
MESSAGING WITH YOUR PROVIDER
You will also be able to exchange messages (text, photo) with your health provider using the “Inbox” section of the app.
