MyCare Virtual by AMC Health

AMC Health created MyCare with you in mind. To use, you must be previously enrolled as an AMC Health monitored patients.

We make it easy to take care of yourself for a healthier you. Your MyCare app works with AMC Health’s Pulse Oximeter, Thermometer, Weight Scale, Inhaler Sensors and Blood Pressure devices. Have your own home health devices? Your MyCare app is easy and flexible enough so you can enter your health vitals using those devices.

We also provide easy access to a variety of tools so you can see how your day-to-day vital measures are trending, from 7 days up to 30 days. All your data is securely stored in one place for easy access and simple-to-understand charts and summaries. You can gather insights, learn, and adapt your lifestyle where and when you need it. You can even get near real-time feedback to help you reach your personal health goals.

Also, your app comes with alerts, support, and guidance. So, whether you are at home or on the go, your clinical team or health coach is there to help you manage chronic conditions like diabetes, heart failure, COPD, hypertension, and weight management.

Need a helpful reminder adhering to your care plan? Your MyCare app is always there to help. Timely reminders can be set up by you or provided by your clinical team (nurse, doctor, health coach) so you stay connected when and where you need it most.

We also offer the option of biometric login, as well as easy access to a variety of tools that include two-way messaging.

Patients who have enabled video within CareConsole can participate on Nurse initiated Video Calls via the mobile app.

Patients can now search for educational video content to support self-care knowledge of their conditions.
