My Heart Visit

It began as a question, “How can we bring patients, caregivers and families together when they can’t actually be together?” After all, people need hope, support, information and resources now more than ever.

The answer is Visit Me—a way of connecting patients and their families with others who truly understand them because they have been there too. We know that these connections can make all the difference.
