Mobile access to the Cesia Continuum™ care management platform from Medically Home. Cesia(TM) Mobile allows doctors, nurses and other caregivers to access their patients from anywhere - including joining video visits, reviewing schedules and uploading images.

Cesia Mobile is not for Medically Home patients. To use the app, you need to be a credentialed user at a health system that uses Cesia to coordinate patient care.
