Guest users (ages 16+) may schedule visits without registering for MaimoCare but cannot reschedule/cancel without registration.

Your E-Forms (electronic forms) are available up to 5 days before your scheduled appointment. Completing E-Forms before your visit can save you time at your appointment. E-Forms are available in 10 languages with the assistance of our multilingual support.

EMERGENCY DEPARTMENT PRE-ARRIVAL: Our Bay Ridge and newly opened Children’s Emergency Departments are ready for you and now you can notify them of your intended arrival time and why you are visiting the emergency department.
