ConnectedCare MH

The ConnectedCare MH app is an easy-to-use remote monitoring solution for patients to share their health and outpatient recovery status in between follow-up visits. Patients are able to self-report their level of discomfort on a scale of mild to severe, as well as their symptoms and health indicators (body temperature, pulse rate, and blood oxygen levels). This allows healthcare professionals to tap into a more holistic overview of the recovery process over a period of time and can personalise the patient's care plan with prescribed medication and exercise.
