Premier Home Dialysis Charting

The Premier Home Charting app allows Patients of Premier Home Dialysis to log their at-home CAPD or CCPD dialysis treatments.

How it works:

-Premier Home Dialysis patients can create a user account to when signing up for the first time

-Patients then select the treatments that they need to log each day

-Patients then fill out a form to chart their at-home treatments. These forms can be filled out for each treatment that needs to be logged.
