MyPD

The MyPD patient mobile application provides peritoneal dialysis clinics and their patients with an interface to collect and view patient therapy exchange and vitals data. If your dialysis clinic is a participating clinic, then your clinic will be able to register you for MyPD and you will be able to use the app to perform the following key functionalities:

• Protect user information by secure account login
• Capture vitals data through Bluetooth as well as manual data entry
• Capture Photos and share with care team
• Capture and view CAPD exchange data
• View APD exchange data
• Automatically share details collected with your care team
• View therapy trends and treatment history
• View prescription details as prescribed by your care team
• Get notified when there is a change to your prescription
