Get your care on the go with the Horizon NJ MyCare app. The Horizon NJ MyCare app is intended for use by Horizon NJ Health and Horizon NJ TotalCare (HMO D-SNP) members in New Jersey.

With the free Horizon NJ MyCare app, Horizon NJ Health and Horizon NJ TotalCare (HMO D-SNP) members can:

• View covered benefits, including medical, pharmacy, behavioral health, dental, vision and more
• View, download or request a new member ID card
• Change your PCP
• Find a doctor, dentist or other health care provider
• View your Care Manager, if you have one
• Find community support and resources available to you
• Learn about additional services available to Horizon NJ TotalCare (HMO D-SNP) members only such as:
> Horizon EXTRA Benefits Card features
> Telemedicine
> And more!

NOTE: If you have Horizon Blue Cross Blue Shield of New Jersey coverage through your employer or directly, download the Horizon Blue app.
